Our Mission

Solace and Sage, a 501(c)3 nonprofit dedicated to helping adults 60 years and older in Central Texas preserve quality of life while aging in place with a chronic, progressive or terminal illness.

Change is a natural part of life, but living with change can be difficult. 

Successfully aging in place requires a whole-person response to change.

As we age, we face more change at a rapid rate, impacting more areas of our lives than ever before.

81% of adults 65 years and above have multiple chronic conditions. Older adults with chronic and progressive illnesses have higher outpatient and inpatient visits and emergency room visits impacting their quality of life and the loved ones caring for them.

Aging in place when living with a chronic, progressive or terminal illness requires a whole-person adaptation process. We recognize that physically adapting to challenges associated with aging in place is not enough. 

Aging in place with peace of mind and confidence requires physically, mentally and emotionally adapting to adversities, especially when living with a chronic, progressive or terminal illness. Learning to adapt through prioritizing your values restores your sense of self and preserves quality of life.

Our Services

We understand the challenges you and your loved ones face as you manage and navigate a chronic, progressive or terminal illness at home. 

Our team of Occupational Therapy practitioners, Nurses, Certified End of Life Doulas and Certified Aging in Place Specialists offer consultative and direct services in the comfort of your home to help you and your loved ones better respond to current and future changes so that you can maintain a sense of control, emotional well-being and competence as you age in place. 

Our whole-person intervention approach involves addressing you as a whole person and considers all of the factors that promote and influence your quality of life and the quality of life of the loved ones supporting you. 

You are more than a diagnosis and more than a body part receiving treatment.

You are more than a patient.

Our services identify and address your needs in 3 main areas of focus:

Personal Needs:  

Are you experiencing challenges participating in what is important to you?

Are you or the loved ones supporting you needing help with: 

  • self-care tasks and routines,

  • home management tasks and routines,

  • finding balance between the demands of a changing and evolving illness and establishing roles, systems and supports; 

  • prioritizing values and preserving sense of self;

  • caregiver training;

  • identifying and expressive end of life wishes

Medical Needs:

Are you or the loved ones supporting you experiencing challenges in overall health management of your chronic, progressive or terminal illness? 

Are you needing help with:

  • Social, emotional and physical health promotion: understanding, communicating and advocating with healthcare and insurance providers 

  • Symptom and condition management: coping strategies, pain management, understanding trajectory of illness, using community and social supports, recognizing signs and symptoms

  • Health and wellness routines: medication and nutrition management, physical activity

Environmental Needs:

Have you fallen or are you afraid you are going to fall?

Do you or the loved ones supporting you have concerns about the safety, accessibility and functionality of your home now or in the future with the progression of your illness?  

Are you needing to discharge home safely from a hospital or inpatient stay following a medical event and need your home modified?

Our team of occupational therapy practitioners are Certified Aging in Place Specialists and experts in assessing your home and providing modifications and recommendations and working with contractors to create spaces that match your current and future needs. 

Ask us about funding that may be available to modify your home!

Our mobile hybrid outpatient part b occupational therapy services are covered by insurance when based on medical necessity determined by your doctor. Services not covered by insurance are based on eligibility for financial assistance and according to a sliding scale and availability of resources.

John Allen McCarty Veterans Scholarship Fund

This fund was established in honor of a real hometown hero who survived the Bataan Death March and 32 months of tortuous captivity. Although he suffered intolerable conditions, he

"...put forth every effort by day and night to improve the morale and well-being of his fellow prisoners of war."

He understood the importance of home and community and the strength and blessings that come from helping others.

It is our wish to carry out his legacy and bless Veterans in need.

Frequently Asked Questions

We understand managing and navigating challenges associated with aging in place can be difficult.

Question 1: Are your services covered by insurance?

Occupational Therapy services that are ordered by your doctor and are considered medically necessary may be covered by Medicare and Medicare. We will be glad to help you in determining if you have coverage for these services.

Question 2: Do you offer a sliding scale?

Yes. Services not covered by insurance are based on eligibility for financial assistance and according to a sliding scale based on federal poverty guidelines and availability of resources.

Question 3: Do you have a lending closet for donated adaptive equipment or frequently needed items such as wipes, briefs, etc.?

We gladly accept donations for gently used equipment for reuse in the community. We can also help you locate equipment that you may be needing from local community resources.
